The first step to obtaining help from the Miami County Cancer Foundation is to download the application. You must be a Miami County, Kansas resident in order to obtain help from our foundation. Once you have completed the application, please hand deliver it to our offices.
Step Two:
HAND DELIVER APPLICATION TO OUR OFFICE
Once your application is complete, please hand deliver it to our office, which is located at 2 W. Peoria, Paola, Kansas 66071.
Our hours of operation are:
Monday, Wednesday, Thursday & Friday
10am to 2pm CST.
